Not the first time and it certainly wont be the last its just part of his game. Talking about a guy with around 150 games for us, 27 years old and still making the kind of mistakes you would associate with an 18-19 year just promoted from the academy. Its never going to change the only way it will get better for the team is to move him on.  Or bring some real competition for his place cause at the moment he's got fuck all to worry about and come Sunday he will be back in the starting 11.
